Systematic review of proton pump inhibitors for the maintenance of healed reflux
oesophagitis

The aim of the study was to compare the efficacy of lansoprazole with all other
proton pump inhibitors in the maintenance of healed reflux oesophagitis through
a systematic review of randomised controlled trials. A meta-analysis of patients
in remission of reflux oesophagitis at 6 and 12 months was performed. The primary
analysis used lansoprazole 15 mg as the common comparator. The results at 6 months
were: esomeprazole 20 mg (Relative Risk 1.09; 95% Confidence Interval: 1.02, 1.17);
lansoprazole 30 mg (RR 1.12; 95% CI: 1.06, 1.18); omeprazole 20 mg (RR 1.12; 95%
CI: 1.06, 1.19). The results at 12 months were: lansoprazole 30 mg (RR 1.12; 95%
CI: 1.05, 1.18); omeprazole 10 mg (RR 0.79; 95% CI: 0.64, 0.97); omeprazole 20
mg (RR 1.19; 95% CI: 1.10, 1.30). Standard-dose PPIs are more efficacious than
lansoprazole 15 mg in maintenance therapy. Esomeprazole 20 mg is the only step-down
dose PPI able to show a significantly higher proportion of patients in remission
of reflux oesophagitis when compared with lansoprazole 15 mg.
