We analysed the general practice DIN-LINK database for a 12 month period ending
March 2003 to establish the level of statin usage in the UK and to estimate the
numbers who achieve target for total cholesterol (TC) and the numbers of coronary
heart disease (CHD) events avoided. 4.7% of the total DIN-LINK sample were prescribed
statins (males 5.4% and females 4.0%). The percentages of individuals without
CHD, with CHD, or diabetes prescribed statins were 2.5%, 49.3%, and 31.1%, respectively.
The proportions of individuals prescribed statins increased with age from 16 to
75 years and then dropped in the age group >75 years. Our model predicts that
(a) less than half of all statin users achieve TC target (b) more than 130,000
CHD events are avoided over 10 years in the UK as a result of statin treatment.
If acute management costs of myocardial infarctions are assigned to the avoided
events, our model predicts a saving of £145 million over a 10-year period. Although
prescribing of statins has increased, many patients who may benefit from lipid-lowering
agents either do not receive them or are under-treated.

The BOMET-QoL questionnaire was developed in three phases. Item generation phase:
where a literature search was performed and 15 experts (10 oncologists, 1 haematologist
and 4 urologists) identified the main domains of impact on health-related quality
of life (HRQoL) in patients with bone metastases (BM), and 15 semistructured interviews
were conducted in patients with BM to identify the domains and items related to
the self-perceived impact of BM. Item selection phase: where the experts scored
items according to clarity, frequency and importance. A preliminary questionnaire
was then developed and self-administered to 92 patients with BM. A final reduction
analysis was performed by conducting factorial analysis and Rasch modelling. Factorial
analysis identified eight dimensions that explained 73.2% of the total variability.
Rasch modelling was then applied by dimension and a 25-item questionnaire showing
good internal consistency (Cronbach’s α = 0.94) was obtained. The BOMET-QoL questionnaire
is a feasible, reliable and specific 25-item instrument for assessing HRQoL in
patients with BM.
